State of Virginia,

County of Henry, To wit: On this the 1st day of January 1862, before me Geo. W. Booker, a Justice of the Peace for the County and State aforesaid personally appeared. Sally L. Tompkins and made oath that she is the widow of Edmund T. Tompkins deceased late a 2nd Sergeant for Capt. Geo. W. Mills’ Company G of the 42nd Regt. of Virginia Volunteers. This affidavit is made in order to obtain the pay &c due the said Edmund T. Tompkins at the time of his death. And at the same time also appeared Wm. A. Campbell and made oath as aforesaid that the facts sworn to are true and Wm. A. Campbell is disinterested therein.

I Jeremiah Griggs Clerk of the Court for the County of Henry and State aforesaid, do certify that Geo. W. Booker where genuine signature is attached to the within certificate was at the time of signing the same, and is now, a duly commissioned and qualified Justice of the Peace for the County and State aforesaid, and all his acts as such are entitled to full faith and credit. In testimony whereof, I have herewith set my hand, and affixed the same of my said Court in office at Martinsville this 12th day of January A. D. 1863.
